Where can I find the Health Assessment Form?
Your sponsor or employer may offer additional Points or rewards for completing a Health Assessment program (separately from the Virgin Pulse Health Assessment available through Wellsource). If this type of incentive is available to you, it will be listed as a reward on the "My Rewards" page or the "How to Earn" page.
All of the details regarding your Health Assessment will be listed on the Company Programs page within the Virgin Pulse member website. You'll have to complete a Health Assessment Attestation form on the website.
1) Log into your Virgin Pulse member website.
2) Click on "Programs" tab in the top navigation menu.
3) Locate the Health Assessment section to view the details. Click on the link to complete the Health Assessment Attestation form (pictured below).
4) Complete the form and click "Submit".
It may take up to 30 days following the submission of your form to view the results in your Virgin Pulse member account. It could also take up to 30 days to receive any associated rewards.
