A biometric screening is a snapshot of your health. Getting tested can reveal health risks early when they are easier to prevent or treat. Results can also be helpful when trying to reach a personal health goal.
Blood cholesterol, blood glucose, and body composition measurements are critical health numbers that can help determine your risk for cardiovascular disease and diabetes. Annual monitoring helps you stay on top of any risks you may have.
Once you download the VP Physician form, have your doctor complete and sign it and submit the completed biometric form to Virgin Pulse to get rewarded!
Submit your biometric form by using one of the following methods:
- Fax to: 1-508-302-0055
- Submit it through your Virgin Pulse account.
To do so, click on the vertical green Support button on the right side of your screen.
On the Support page select Send a Form.
Then choose the Member Inquiry option from the drop-down menu:
Fill out the necessary information and upload the biometric screening form under Attachments.
Notice: Your Biometric Screening form should be downloaded from your own wellness website account. Your form includes a unique Member ID that is used to make sure you receive credit for completing this activity.
It may take 7-10 business days once the form is returned for your reward to be credited to your account. Your Biometric screening results will be available on the VP app for three months after you receive them. To check your results from previous months or years, you may need to access the Stats page on your Virgin Pulse website account.
