My Care Checklist helps you keep track of your preventive exams, all in one place. You'll earn rewards for staying up to date, plus reminders to keep you on track! Some exams are based on your health situations (i.e. age, gender, and health conditions). You can update these anytime.

You can earn points for completing the My Care Checklist preventive exams. by a simple self-attestation of the date, exams were completed. Or if you’re on the PPO plan, completed exams can validate automatically in our system. This can take up to three months – so plan ahead!

What happens to My Care Checklist rewards when a new PATH Program year begins?

When a new Program year begins, it will be ensured that any My Care Checklist points are added to your account if you are still in compliance. For example, if you received your flu shot on Sept. 21, 2022, you will receive points at the start of the 2023 program since it’s within a year and you’re still in compliance. If you don’t receive your flu shot in 2023, at the start of the new program year you’d be considered out of compliance and would not receive points in 2024.
